Since this is a common question, trying to answer it for all.

Once you land in Japan, complete your paper work, PCR testing, app installation, a quarantine hotel is provided by the government. You do not choose it or pay for it. It could be a bit away from your airport of arrival depending on the availability of rooms.

After the institutional quarantine at the hotel, you will be retested and dropped back at the airport of your arrival. From there, you cannot take domestic flights or normal taxi to reach your next destination.

Only a bogie in the airport trains is arranged for travel up to few key stations. Limousine buses are available until certain hotels if you have a booking there. Rest, either arrange for a private car, or stay near the airport in case your home is too far or you wish to do so for safety of others.
